Fourth Street Clinic

Fourth Street Clinic, located in Salt Lake City, Utah, is a Federally Qualified Health Center (FQHC) funded through the Health Resources and Services Administration, of the United States Department of Health and Human Services. Fourth Street Clinic is designated as a homeless health center which mandates that at least 75% of our patients are homeless.

Founded in 1988, Fourth Street Clinic was a response to the unmet medical needs of the growing homeless population in Utah. Initially operating as a one-room triage clinic with a part-time nurse, the clinic relied on local hospitals for patient treatment. In 1993, Fourth Street Clinic relocated to its current facility at 409 W 400 S, Salt Lake City, and attained nonprofit status in 1998.

Since then, the clinic has expanded its services to enhance integrated, comprehensive care for this underserved population. Significant milestones include the opening of an onsite pharmacy in 2001, the introduction of behavioral health services in 2011, and the launch of an onsite dental clinic in 2014. In 2019, Fourth Street Clinic further broadened its reach by introducing a Mobile Medical Clinic and expanding the Medical Outreach Street Team, reinforcing its commitment to providing accessible, low-barrier health care. Today, Fourth Street Clinic employs over 135 dedicated professionals and operates with an annual cash budget of $19.1 million.

Over the past 38 years Fourth Street Clinic has exemplified its mission through addressing the health disparities, and complex needs of individuals and families experiencing homelessness by providing comprehensive medical, behavioral health, dental and other enabling services through an integrated care model utilizing the principles of Trauma Informed Care. In 2025, we served 5,754 unique individuals, including children, 93% of whom were currently experiencing homelessness through 29,438 patient visits. Our pharmacy played a critical role in this care, dispensing 103,600 prescriptions last year.

As a Federally qualified Health Center, Fourth Street Clinic serves patients regardless of their ability to pay. Health insurance is accepted but not required. Fees are based on a sliding scale tied to the Federal Poverty Level (FPL), with no charge for those below 100% FPL and a $1–$4 fee for those between 100% and 200% FPL. In 2025, 82% of patients had income below 100% FPL. 

Fourth Street Clinic carries designations from the Health Resource & Services Administration for Health Professional Shortage Area in Dental Health (ID: 6499994947), Mental Health (ID: 7499994929, Primary Care (ID: 1499994970) and has a medically underserved population (MUP) designation (ID: 07737) for primary care to individuals who are included in the Federal Government’s Low to Moderate Income category.

"Fourth Street Clinic has helped me a whole lot. I've been here many times for alcohol sickness, Alcoholics Anonymous, and I come in for therapy every once in a while," Nathan says. He is now living in low-income housing and is 18 months sober.

"I think Fourth Street Clinic provides excellent service; everyone treats me with dignity. I absolutely have benefitted from the dental clinic — I can smile again!" Kim, a dental patient, says. Her new smile gave her the confidence to land a full-time job and is now living in an apartment outside Salt Lake.

"One of the things Fourth Street Clinic has been great about has been to take care of my daughter's health. The clinic has been outstanding in getting us the medication she needed to help herself in her own situations," Maria said. Maria and her daughter became homeless when they fled from an abusive relationship. They are now living together and are breaking free from homelessness.
