Child Health Investment Partnership

Child Health Investment Partnership (CHIP) is an early childhood home visiting program serving children and their families living in Bedford, Botetourt, Craig County, Roanoke and Salem. Founded in 1988, CHIP partners pregnant mothers, children birth to kindergarten entry, and their caregivers with a Community Health Nurse and Family Case Manager to assist with health care coordination, family strengthening support mental health counseling and kindergarten readiness. 

CHIP services emphasize prevention and early intervention, when preventive services make the greatest impact in the life of a child. CHIP offers comprehensive family support including:

Health Care Coordination

• Prenatal case management

• Developmental screenings

• Nurse case management for chronic and medically complex conditions

• Oral health education and application of fluoride dental varnish

• Assistance with establishing a medical and dental home

• Referrals for primary and special needs care and care coordination with health providers

• Obesity prevention and nutrition education, including lactation support

Mental Health Services

• Access to a dedicated Licensed Clinical Social Workerfor CHIP enrolled children and their care givers

• Play therapy, traditional talk therapy, group counseling, and more

Family Strengthening

• Monthly education on topics such as family planning, safe sleep, immunizations, home safety and parenting skills, tailored to the unique concerns of each family

• Support in setting goals based on family strengths

• Referral services to assist families in obtaining community resources such as food, housing, employment resources and social services

• Follow-up to ensure that referred services are obtained and meet current family needs
