Coordinating Center for Home and Community Care Inc

This is about Sarah, a young woman who resided in a nursing home for close to two years.  She was recovering from a devasating accident that took her daughter's life and kept tSarah comtaose for months.  At the time of the accident, she had just given birth to a little boy. While Sarah was in the nursing home, she not only lost custody of her surviving children but also any meaningful contact with them.  As she struggled to reclaim her life she learned about The Coordinating Center and a Maryland Medicaid program that would help her regain her independence, and  assist with locating the essential services she had to have to stay healthy and safe in a new home.  In Ms. S’s words, “The waiver was the best thing that ever happened to me.” 

Through The Center's Housing Office she was also connected to help in finding and paying for a home for herself and her little ones.  As Sarah describes her home, funded through a special HUD voucher program, she says its as "good as anything Donald Trump ever had”. new home was the last thing she needed to succeed in getting her children back at home with her.  Those two little ones  andn the dreams she has for them are the reasons for her continuing efforts to walk again and to find creative ways to be the full time mother they need her to be.

The Coordinating Center serves many "Sarahs" whose lives were shattered by trauma, or who live daily with the most challenging health and disability concerns.  These are people across the spectrum of disability, those of all ages from little ones to older adults.  They  give deeper
