Good Samaritan Clinic

Good Samaritan Clinic provides primary care with a focus to the uninsured on prevention and treatment of chronic diseases. In 2023,  98% of our patients were diagnosed with either diabetes, hypertension, or high cholesterol with many being comorbid with more than just one. 

Patients are provided diagnosis-specific health education and a health care plan. Our community health worker follows up on those diagnosed with a chronic disease to ensure that the patients are following the care plan as determined by the provider including compliancy with the prescribed medications, following through with lab orders and referrals to specialist within our referral system. Our Director of Programs ensures that our systems are designed to make it easier for the patient to navigate the health care system.
