ABOUT US:  Hill Country Health and Wellness Center changes lives by providing essential healthcare, education and support to a rural communities in Shasta County, California. We work in partnership with our patients to find the right care, providing hope and support and connection as they find what they need to live whole, healthy and satisfying lives.

From our inception, Hill Country has been a beacon of hope in our region, tirelessly working to provide the highest standard of healthcare services to every resident, regardless of their background or circumstances. We believe that healthcare is more than just a visit to a doctor but involves looking at each person from a holistic perspective. We’ve discovered that when we take this integrated approach to care, it is much more effective at creating the change that people want in their lives, especially those facing the most difficult circumstances.  

CHALLENGE:  Providing integrated care and helping patients navigate the healthcare system is a complex endeavor, made even more challenging in a rural community. The scarcity of resources, such as housing, lack of healthcare infrastructure, and geographical isolation often result in disparities in access to quality care.   Every day people show up at Hill Country's different locations , looking for food, shelter, and help for untreated problems that have become severe.  When they are met with respect by a helper who listens to their story and provides concrete assistance, change is possible.  Unfortunately, our health care system is not yet designed to pay for the kind of care we know works.

HOW YOU CAN HELP:  With all of these challenges in mind, Hill Country finds itself in a pivotal position. Current funding streams are simply not keeping up with the expenses necessary for our unique approach to effective integrated care.  We need your help to continue the work we are doing.  Your donations will allow us to continue this work of providing whole person care while healthcare catches up to this approach to care and provides a sustainable payment model to catch up.  It will put financial resources under the positions that are currently not reimbursable by insurance but are essential to helping people in our community to access their healthcare and achieve their wellness goals.
