At the Holyoke VNA Inc., Hospice Life Care, our team of professionals works with you and your physician and family to develop a plan of care which allows you to remain safely in your home.  Our home care services include skilled nursing, physical, occupational, speech and vestibular therapy as well as social work, home health aides and a dietician.  We offer telehealth monitoring and other programs that allow for patients to remain out of the hospital and comfortable at home. 

Our Hospice Life Care staff provides compassionate and holistic care focused on managing symptoms, keeping patients as comfortable as possible and helping patients to live out their last days with quality and dignity.  Our Hospice program offers complementary therapies such as music therapy and aromatherapy. We have a specialized Medical Director as well as a Chaplain, bereavement coordinator and many volunteers.  Our program offers 13 months of bereavement support after the passing of a loved one. 

We are also a part of the "We Honor Veterans" program and are fortunate to have a Veteran Liaison on staff, who is a Vietnam veteran himself.  He is very active within the veteran community and works closely and collaboratively with local and state veteran resource services to support our veterans and their families at the end of life.

We have served our communities for over 100 years and remain a home care agency of choice in Hampden and Hampshire counties, soon to include portions of Franklin County.
