Most of us take for granted the myriad little tasks that get us up and out of the house each monring.  But what if you could no longer get dressed by yourself or cook a meal or lift the laundry basket?  Many seniors and adults with disabilities face these challenges all the time.  Home Care Partners is there to help.

We send a professional case manager to create a personalized plan of care for each client and then schedule a trained aide to do those tasks that clients can no longer do on their own.  We work in partnership with local area agencies on aging and the Veterans Administration.  Our aides work in DC, Arlington, Montgomery and Prince George's Counties.  We also work with family caregivers providing educational information and respite support to help them with their responsibilities.  Many of our aides are graduates of our own licensed training school, and all our aides receive ongoing continuing education at our school each year to keep their skills up to date.  Our staff---case managers, aides, nurse educator, and schedulers---work as a team to make sure the best care is provided.
