Housing 4 the Homeless serves San Diego’s most vulnerable unsheltered community members helping them move from crisis and vulnerability to safety, stability and self-sufficiency, by connecting them to life saving services & community.
Our objective is to help participants stabilize their physical and mental health and find a permanent housing solution. We support the individuals experience of autonomy, competence, and dignity, restoring their connection to a hopeful and thriving future.
H4H is a vital service provider, partner, and collaborator serving an unmet need for medically fragile unhoused older adults, individuals who are terminally ill, and single parents with young children. Many are from marginalized groups that have historically suffered injustice due to their race, disability, and socio-economic status. H4H has found these vulnerable individuals and families aren’t resistant to services, they are resistant to programs that don’t serve their needs.
Using a person-centered homelessness response, H4H employs a trauma-informed approach focused on equity, progressive engagement, and harm reduction. Participants are identified on the streets generally during late night hours (when other services are closed) in known locations, like outside of hospitals having recently been discharged, with nowhere to go. The participant is connected with an advocate case manager to help them stabilize, connect to services and ultimately to a permanent housing solution.
