I4A is working with Americorp Vista volunteers to develop at least 3 strong programs in member agencies to meet the needs of our senior population.  The first program is a hospital to home care transition project called GRACE (Generational Resources and Assistance through Community Engagement).  In this program, patient coaches work with high risk older adults.  Project GRACE seeks to build a kind and capable care network by equipping volunteers with successful coaching skills directed at avoiding hospital and/or long term care readmissions.

A second program, the Friendship Corps, promotes public awareness of the needs of elders in the community and works to link them to community resources while promoting an environment where volunteers and participants share their gifts and form relational bonds.

The third program is Seniors Assisting Seniors (SAS), a volunteer transportation program that provides free transportation services to frail and elderly seniors with no other means of transportation.  The volunteers provide transportation to healthcare appointments, pharmacies, groceries, and personal appointments and visits.  SAS provides "door through door" service to our clients.
