A significant association exists between unmet social need and poor health. When social needs (housing, food security, medication, transportation, preventive care) are left unmet, no amount of healthcare intervention will improve community health or the quality of life.  Without the knowledge of options or means of accessing services and care; our clients are referred to us because they utilized the 9-1-1 and Emergency Medical services for access to the Hospital Emergency Departments, where they are never turned away and their immediate need is only temporarily met.  These service are not intended to be our community’s primary access point to healthcare (unless it is emergent) and social services, and are not designed to effectively mitigate these needs.  Properly navigating our community through the resources appropriate for their needs results in a reduced cost of healthcare and an improvement in community health and quality of life.
