Lifestyle Hospice Foundation Inc

The Lifestyle Hospice Foundation was established in 2004 and are a registered 501(c)(3) non-profit organization.  

The Foundation’s purpose is to provide financial assistance for hospice services and durable medical equipment and supplies to terminally ill patients and their families for whom other resources are limited. 

Sometimes the service of a hospice patient involves granting a last wish.  This may consist of giving them the opportunity to see loved ones one more time, or just getting to do something they love one more time.  Granting a last wish can sometimes make a big difference in providing for a peaceful passing, not only for the patient, but for their family as well. 

All these services are made possible by the generous donations of many people and organizations throughout the community.

$10.00 Walking Aid for Hospice Patient (i.e. cane/walker).

$25.00 One hour of RN Nursing Care

$50.00 One week of CNA nursing care.

$100.00 One month of Hospital Bed or Oxygen in-home rental.

$150.00 Wheelchair purchase.

$250.00 Hospice medicines for a week.

$500.00 Lift chair purchase

$1000.00 Building lease payment

$2000.00 One month of full-time administrative staff coverage.

$2500.00 Pay for the last wish of a hospice patient.
