Mountainlands Community Health Center Inc

Mountainlands Community Health Center (MCHC) is dedicated to providing affordable, comprehensive health care services including family practice medical, dental, mental health, and pharmacy services to Utah’s low-income, uninsured, and underinsured populations.  MCHC is the sole comprehensive primary care provider in Utah and Uintah Counties utilizing a sliding fee scale based on a patient’s income and family size.  We continually seek additional funding partners who are interested in helping us remain the most cost effective provider of primary health care in Utah and Uintah Counties.

MCHC currently has five clinic locations.  Our main site, including administration, is located at 589 South State Street, Provo.  Our other clinics are located in Vernal, Payson, one co-located within Wasatch Mental Health in Provo and another co-located with the Food and Care Coalition in East Bay Provo.

The Mountainlands East Bay Health Center in Provo is the first and only health center in Utah County to provide primary care services to individuals and families experiencing homelessness.

In 2016, MCHC served 15,338 patients through over 44,000 patient encounters.  Over 68% of patients earned less than 150% of the Federal Poverty Level.  69% of patients were uninsured (nationally health centers see an average of 36% uninsured patients).  Our patient population includes migrant and seasonal farm workers, homeless, and HIV positive individuals.  MCHC patients are ethnically diverse with over 72% best served in a language other than English.
