How do you wish to die? That is an important question and as Baby Boomers advance in age the question becomes more pressing, not only for the Boomers, but for others near the end-of-life and their families. Do you want to die peacefully supported by friends and family where you live? Or, do you want to fight the good fight to the end, perhaps hospitalized with all that modern medicine can offer? The decision should be yours; they are entirely different approaches to death and we all may choose differently. But, who would know and how assured are you that your wishes will be followed?

Nevada POLST is an entirely volunteer organization promoting the POLST Program for those near the end of life. The POLST is a form that translates patient's wishes onto a medical order that is to be honored in all settings: home, nursing home or hospital. The bright pink form travels with the patient if transferred and is easily recognized in case of emergency.

Nevada POLST provides the Nevada State POLST form, along with free training to all healthcare providers in Nevada and education to community groups wishing more information.
