Hospice Care

Haven offers compassionate care and support to adults and children with life-limiting illness and their families, which address the physical, psychosocial, emotional and spiritual needs associated with advanced illness while respecting each individual’s goals, beliefs and wishes. Haven’s team of medical providers and staff will expertly manage pain and symptoms at home or in a variety of settings including hospitals, nursing homes or assisted living facilities.

In addition to providing comfort through compassionate delivery of hospice services to adults and children, Haven offers Transitions, Palliative Care Consultations and Advance Care Planning with a patient-centric focus.

As a community-based, not-for-profit hospice, we are committed to providing our programs and services regardless of a patient’s ability to pay. One hundred percent of the gifts and donations made to Haven remain in our communities and help provide care for patients unable to pay, as well as the many unfunded programs and services we offer. Our donor-supported programs and services include:
