Nurse-Family Partnership's maternal health program introduces vulnerable first-time parents to caring maternal and child health nurses. This program allows nurses to deliver the support first-time moms need to have a healthy pregnancy, become knowledgeable and responsible parents, and provide their babies with the best possible start in life. The relationship between mother and nurse provides the foundation for strong families, and lives are forever changed – for the better.
Each vulnerable new mom is partnered with a registered nurse early in her pregnancy and receives ongoing nurse home visits. It is a life-transforming partnership, for the mom and her child. Typically, nurses visit families from the mother’s first trimester until her child’s second birthday. This early intervention during pregnancy allows for any critical behavioral changes needed to improve the health and welfare of the mother and child.
Nurse-Family Partnership is a national program serving mothers and their children in 43 states, the U.S. Virgin Islands and six Tribal communities. Currently, there are 187 families receiving nurse home visits in Santa Clara County and 125 families in San Mateo County.
