Park DuValle Community Health Center Inc.

Park DuValle Community Health Centers is seeking community support for the Dr. Harvey I. Sloane Patient Assistance Fund.  Each year, PDCHC performs thousands in uncompensated care services that cannot be afforded by our patients.  We offer low-income patients a sliding fee scale discounted services based in income and family size.  Often the nominal fees can prove to be a barrier to care. This burden is even more exacerbated when patients have multiple medications.  More than 70% of our patients have two more chronic diseases. Together, we can bridge the gap of health and social determinants in the communities we serve!

Donations to the Dr. Harvey I. Sloane Patient Assistance Fund will assist in relieving the burden of medication costs and avoid the unsafe habit of picking and choosing which medications to fill based on what they can afford.
