Peninsula Healthcare Connection promotes integrated primary, mental health care, and intensive case management for homeless and low-income individuals in Santa Clara County, regardless of their ability to pay.  PHC collaborates with local hospitals, healthcare providers, social service agencies, and similarly committed partner agencies to achieve this goal.

PHC HAS TWO CORE SERVICE AREAS:

PHC Clinic: The PHC clinic is located within the Opportunity Center in Palo Alto.  Homeless and low-income individuals can receive the following care at the PHC clinic free of charge: primary care, psychiatry, dermatology, lab testing, vaccinations, access to prescription drug programs, and referrals to dental and specialized care.  Patients are seen by highly trained and qualified Palo Alto Medical Foundation Medical Doctors who staff the clinic, and by a registered LVN.

New Directions: New Directions is an innovative hospital and community case management program that operates within Santa Clara County.  Frequent users of the hospital and social service system are referred for case management, with a goal of reducing the number of emergency room visits and in-patients nights.  Intesive case management services are delivered by highly trained and educated social workers, assisting their clients with returning to self-sufficiency.
