Precision Healthcare Ecosystem

Precision Healthcare Ecosystem (PHE) is a CA 501c3 nonprofit corporation, created with the vision that "The Doctor of the Future is One's Self.”  Its inaugural program, Project Apollo, utilizes a multi-disciplinary, collaborative and integrative care model to educate, enable and empower participants to lead a personalized health journey.  Members of our founding cohort are guided by their own quantified data to achieve a bold Vision and Mission:

Mission – to transform healthcare by amplifying patient and community voices through data-informed processes toward a collaborative care model.

Purpose – To help people feel seen, heard, and respected so they feel empowered to take ownership of their health.

The Project Apollo founding cohort includes an extraordinary group of professionals who are also patients with hard-to-treat, undiagnosed or other challenging medical conditions.

We at Precision Healthcare Ecosystem are:

• Elevating and validating patient experiences and voices; 
• Providing practitioners with increased visibility to both quantitative and qualitative measures of patient care and performance - improving connected care;
• Adequately leveraging evidence-backed Quantified Self biometrics to inform patient care, educate the patient, and 
• Empowering patients to take greater ownership of and responsibility for their health and health data, and advocate for themselves (recognizing that the patient is the paying customer);
• Transforming patient/provider relationships to forge a path toward a new collaborative care model. 

We recognize that patients are exhausted by their search for solutions, for treatments that actually work.  Project Apollo utilizes a whole health history model, grounded in community, and provides educational tools and vetted, specific self-study protocols to support each participant's unique health challenges.  With these tools, participants can collect, observe and get help to interpret their own data, and bring actionable information to their providers for more collaborative care.

PHE's Empowered Patient Toolkit currently exists as a series of work-in-process elements which we seek to assemble into a mosaic of ready-made, effective and proven actionable elements that any patient – regardless of their means or condition – can use to find their voice and better navigate their care.

Empowered Patient Toolkit will contain:

– Timeline: a tool to visualize the patient's lived experience of their condition in context.

– Empowered Patient Playbook: documented lessons learned by empowered patients to help others better navigate their journey.

– Online education for healthcare digital literacy: content to enable informed self-study and identify accurate, actionable information online.
