Every child deserves to feel safe, supported, and seen.
But here in our community, many youth are facing moments of crisis, uncertainty, and overwhelming challenges—often without the support they need.
That’s where we step in.
Through our Community Outpatient Center, SAYS provides coaching, counseling, and care that meets youth and families where they are—at home, at school, and in the moments that matter most. We show up during crises, walk alongside families through challenges, and help young people move forward with confidence and hope.
Because of this work, over 800 individuals were served in 2025 across St. Johns, Flagler, and Putnam Counties.
And the need is growing.
As Northeast Florida continues to expand, more families are reaching out for support. In 2026, our focus is to grow awareness and funding for our Community Teams—ensuring that when a child or family needs help, someone is there to answer the call.
How Your Support Makes a Difference
Mobile Response Team (MRT)
When a youth is in crisis, every moment matters. MRT provides free, 24/7, on-site crisis response within 60 minutes, helping stabilize situations, support families, and connect youth to care.
Since 2015, MRT has completed 2,700+ assessments, diverted over 80% of youth from Baker Acts, and saved our community more than $10 million in hospitalization costs.
Community Action Team (CAT)
CAT supports youth with significant mental health needs through intensive, in-home therapy and wraparound care. Using evidence-based practices, this program strengthens families, builds life skills, and provides a safe alternative to out-of-home placement.
Coaches
Our Coaches walk alongside youth and families, helping them navigate challenges, access resources, and build a path forward. Through personalized, team-based support, they empower lasting, positive change—often preventing the need for higher levels of care.
Care Coordination
Care Coordination connects youth in the child welfare system to essential services, including mental health care, education, housing, and community support. This work ensures continuity of care and helps create stability during critical life transitions.
