Sourcewise

Since 1973, Sourcewise has provided services and support to seniors in Santa Clara County. As Sourcewise, we continue to support seniors and caregivers as we expand our reach to include persons with disabilities and all adults in Santa Clara County. We aim to inform, educate, and prepare all adults within Santa Clara County to better navigate their health and life options.

The Sourcewise mission is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.

Who We Serve

As a provider of crucial services, Sourcewise aims to service all adults 18 years and older, as well as the family members of the individual needing services, and caregivers who dedicate themselves to tirelessly care for family and loved ones – individuals who also need a different set of support services.

In Fiscal Year 2014/2015, Sourcewise provided the following services to individuals in Santa Clara County: 

Community Resource Referrals -- 13,500 individuals served

Meals on Wheels -- 675,000 meals delivered

Senior Employment Services -- 180 individuals served

Health Insurance Counseling & Advocacy -- 26,000

Care Management -- 439

Family Caregiver Support -- 44

What We Provide

From over-the-phone community services navigation to in-person customized guidance through an array of resources and options, Sourcewise offers excellent customer service to all individuals seeking support services. 

Our direct support services include:

• assistance with understanding and obtaining community resources (Information & Awareness)
• navigation of the Medicare system (Health Insurance Counseling & Advocacy Program)
• administration of nutrition to homebound individuals (Meals on Wheels)
• support with updating employable skills and finding a job (Senior Employment Services)
• facilitation of a full-scale care plan (Care Management)
• coordination of temporary respite for caregivers (Family Caregiver Support Program)

Sourcewise provides additional support to the community by funding programs at other service providers throughout Santa Clara County. For Fiscal Year 14/15, Sourcewise awarded monies to organizations providing services in the categories of Adult Day Clubs; Health & Nutrition; Home Care; Care Management; Caregiver Resources, Education, and Training; and Transportation.
