Start Community Health Center is a not-for-profit Federally Qualified Health Center (FQHC) committed to providing services to our community and surrounding parishes.
The overall goal of the Start Community Health Center is to improve access to quality and comprehensive health care services, improve the health status of our patients through prevention, early identification, and treatment of serious health issues. The health center's service model will integrate preventative and primary medical care, dental care, mental health, and substance abuse treatment, case management and other enabling services. Based upon screening and requested needs, clinic staff will provide general preventative and primary medical care on-site. In order to provide all services, we will be working closely with other entities in the community to ensure a continuum of care approach.
Housing Support Team/Community Based Recovery Services is community based supportive services program that is provided to people needing supports to live independently in the community. In addition to the Community Based Recovery services (Services Component), participants may receive a housing voucher or subsidized housing unit as part of their individualized service package. This program will utilize Housing First and community-based services principles. Start Corporation's philosophy is based on the belief that recovery is strengthened and supported by having safe and stable housing, access to basic needs, being a part of a supportive social network, and having meaningful interests and activities. Our goal is to help the persons we serve obtain these things and to help them feel like productive members of their community.Persons/Households with behavioral health, physical health, or developmental disabilities and that meet income guidelines and have a payer source for services.
Supportive Housing Programs: Utilizing a combination of Assertive Community Treatment and Housing First principles, this program provides assistance in locating and securing safe and affordable permanent housing for persons who are homeless and disabled in Terrebonne and Lafourche Parishes. The services provided in this program include, but are not limited to: skills training, locating housing, financial support, and other services necessary in maintaining permanent housing. Referrals can be made directly to Start Corporation by other agencies or as a self-referral. This program is funded by HUD McKinney grant funds and community donations. (Capacity: Program can assist up to 24 individuals or families)
Safe Start:This program provides assistance in locating and securing safe and affordable permanent housing for persons who are homeless, disabled, and victims of domestic violence in Terrebonne, Lafourche, and Assumption Parishes. The services in this program also include, but are not limited to: skills training, locating housing, financial support, domestic violence counseling, legal services, and other services necessary in maintaining permanent housing. Referrals can be made directly to Start Corporation by other agencies or as a self-referral. This program is funded by HUD McKinney grant funds, Region III Office of Mental Health, and other community donations. Start Corporation works in conjunction with The Haven and Chez Hope, two domestic violence service providers in our area, to successfully assist people in this program. (Capacity: Program can assist up to 12 individuals or families)
Youth Build:
EDUCATION
To acquire literacy and numeracy gains, secondary education credentials, industry-recognized credentials, and preparation for post-secondary career and education placements and completion. This component consists of: Academic Skills, Remedial Education and HiSET Preparation, College Readiness, and Career Exploration.
CONSTRUCTION
Provides on-site skills training through well-supervised housing rehabilitation or construction work and leads to industry-recognized credentials for young people. This component consists of: Onsite Training, Health and Safety Standards, Industry Certifications, Career Readiness Skills and Education and Pre-apprenticeship programs.
