SteppingStone (formerly North & South of Market Adult Day Health), is a 501 (c)(3) non-profit organization that operates four Adult Day Health Care Centers that serve residents of San Francisco. We are the oldest and largest provider of adult day health care in the city.
We serve hundreds of frail elders and adults with disabilities each a year all facing medical challenges so serious that they are at risk of no longer being able to stay in their homes. All are low-income residents with over 95% receiving managed care insurance-MediCal. They have no out-of-pocket costs for our services whatsoever. All attend the Centers multiple times each week; many every day.
At our four Centers clients receive skilled nursing, medication management, physical and occupational, therapies, social work services as well as a hot nutritious lunch social and an opportunity to socialize. For most clients we provide round trip transportation as well. Our expert staff is multilingual and able to provide culturally and linguistically appropriate care for our very diverse participant base.
SteppingStone's program of robust adult day health care is effective with clients on average remaining healthy enough to stay in the program for four years and some for over a decade. Considering the medical challenges our clients face this a long period of time to remain in their homes and with their loved ones.
With the advent of the COVID-19 pandemic and the shelter-in-place orders SteppingStone had to suspend care activities at our Centers so immediately implemented a "Centers Without Walls" program. This allows us to continue to provide uninterrupted care for our clients, who are at risk of falling ill without ongoing health monitoring and care, as well as being at high risk for the coronavirus.
With the Centers Without Walls program our clients are receiving care from the same staff they know well from the Centers, many of who have been caring for them for months or years. This continuum of care is important during this health emergency.
SteppingStone is licensed by the California Department of Health Services and certified as a Medi-Cal provider by the California Department of Aging.
