  Help in identifying home-care providers and adult care facilities
  Provide assistance to senior citizens encountering problems with hospital discharge requirements
  Assist seniors in locating medical services
  Arrange for delivery of prescription medicines for homebound senior

 

• Transportation    
  Provide transportation to medical facilities, shopping and recreation centers for senior citizens who are no longer able to drive
  Assist "qualified' senior citizens in applying for and obtaining a pass for Paratransit services
  
       
• Handyman Program 
  Change hard to access light bulbs
  Replace broken cabinet door hinges
  Change smoke alarm batteries provided by homeowner
  Replace outside address light bulbs
  Change furnace filters
  Change rollers in drawers and window

 

• Social Services 
  Upon request and in association with "Caring Callers" call and visit homebound seniors to verify their well being
  Identify sources to assist in solving individual questions regarding Medicare and Social Security
  Assist mentally confused senior citizens
  Provide volunteer shoppers for homebound seniors
  Refer seniors to appropriate government agencies for the specific help they need
  Provide assistance with financial management
  Provide home maintenance referrals
