Skilled Nursing Services


If you have had a recent medical procedure, illness, or injury, are living with a new or chronic condition, or need support navigating the complex healthcare system, TCNA can help you reach your recovery goals with skilled nursing services including:


 



  • Monitoring symptoms and working together with your providers


  • Helping you understand your medications and/or health condition


  • Safety assessment of your home and loaning of equipment to support your independence


  • Wound care after surgery


  • Removal of stitches or staples


  • Dressing changes


  • Blood draws for lab work ordered by your physician


  • Therapeutic injections


  • Referrals to other healthcare providers 


  • Emotional support during life transitions


  • Caregiver coaching



 


Skilled nurses work in partnership with the other members of your medical team and collaborate with you to create a plan of care based on your goals. 


 


How is TCNA different from other Visiting Nurse Agencies? 




  • TCNA does not bill for services


  • Individuals receiving skilled nursing services from TCNA are not required to be homebound


  • TCNA can provide longer term services. In fact, TCNA often sees individuals after they are discharged from other VNA’s but still need skilled nursing services, (like medication management), to remain independent at home. We work in partnership with local VNA's who frequently refer to us to fill this critical gap once Medicare covered services have ended. 


  • TCNA does not provide Physical Therapy, Speech Therapy, Occupational Therapy 



  • TCNA does not provide Home Health Aides

    Hikes with Friends

    Started as a response to isolation during the pandemic, TCNA's Hikes with Friends continues as a way to stay active, foster friendships and get out into wilderness, all things proven to improve our health. For more information on our next hike, contact Ed Parsons at 603-960-0363.


    Meals on Wheels


    Meals on Wheels is a free service. Tamworth residents over the age of 60 qualify to receive meals automatically.  Folks under 60 who find it difficult to leave home, or have difficulty making their own meals at home also qualify. Call 603-323-8511 to find out more and sign up.

    All Persons Trail Project


    Tamworth Community Nurse Association (TCNA), in collaboration with the Chocorua Lake Conservancy (CLC), is working on the construction of the first All Persons Accessible Trail in Tamworth.

    The trail is situated on conservation land owned by the Korson family and it is named the Mary Mullarkey All Persons Trail in honor of Tom Korson's late wife and Andrew Korson's mother, who lived with multiple sclerosis for many years before her passing in 2021.

    The trail spans 0.5 miles round trip and will include compacted stone dust mix and boardwalk sections with railings over wet areas. Additionally, interpretive signs will highlight special features along the trail, serving as educational tools.
