The team at Twinepidemic aspires to create sustainable communities by preventing the epidemic of diabetes and heart disease.

PROJECT GALVANIZE HEALTHY LIVING is a community screening and preventive education program designed to improve early identification of people who are at risk, early risk assessment at primordial , primary, and secondary levels and early implementation of healthy lifestyle changes, timely referral into the health system for treatment, and a reduction in the burden of disease , associated complications and health care costs.

We plan to implement this through Point Of Care Health screening Programs in multiple places in Arizona in a Health Fair like setting. Some of our goals are to :
1) Analyze and describe the prevalence of risk factors and cardiometabolic renal disease states in populations and compare differences between different ethnicities ,other demographic variables, and biomarkers
2) Initiate preventive health education that informs patients about shared risk factors,
3) Enroll in evidence-based self-management workshops
4) Provide access to care, avail of new technology, affordability to medications
