Our Mission:

People are important to the last moment of their lives.  Visiting Nurse & Hospice Home provides compassionate medical care and emotional and spiritual support to those entering the last stages of their lives and to their loved ones who go on living.

Our Continuum of Care:

Visiting Nurse & Hospice Home is a locally-based, not-for-profit agency that provides compassionate, specialized, end-of-life care to patients and support to their families. The hospice program employs an interdisciplinary team (medical directors, nurse practitioner, nurses, home health aides, social workers, non-denominational chaplains, counselors and volunteers) to care for patients and to assist families at the end of life. Our team provides hospice or palliative home care in the patient’s home, in the agency’s 14-bed Hospice Home or in a long-term care facility. After the death of the loved one, bereavement counselors follow and support the spouse or primary care-giver for thirteen months or longer. The majority of patients reside in Allen County but the agency serves patients in eight counties in northeast Indiana.
