Mission

Our hearts, skills, and resources are dedicated to delivering outstanding home and community-based health and hospice services that enrich the lives throughout our region. We do this in active partnership with other organizations and with the individuals and families we serve.

Programs

HOME HEALTHCARE (nearly 10,000 patients in 2004)- Short-term care spans, on average, a 90-day period and includes care for acute illness or injury, rehabilitation post-surgery or injury, and intravenous therapy. Long-term care services help individuals who would otherwise need nursing-home care remain at home. HOSPICE - Nursing & physician care; pain management; emotional & spiritual support to patients, their families, and their caregivers; bereavement support; respite for caregivers; crisis intervention and support in schools; children's services; public & professional education FAMILY SUPPORT SERVICES - The VNA & Hospice serves nearly 1,000 babies, children, mothers and fathers each year through the following programs: Maternal and Child Health, Wellness Clinics, NH Family Support Services, NH Fatherhood Project, NH Women, Infants and Children (WIC)
