White Mountain Community Health Center

White Mountain Community Health Center fosters community-wide health through
quality and compassionate services. These include primary care for all genders
and ages, dental care, substance use disorder treatment, dietician services,
sexual and reproductive health service, a teen walk-in clinic, behavioral
health care, and care coordination. We make sure that everyone in Mount
Washington Valley can access these services through a generous sliding fee
scale and other programs that remove barriers to care.

In
April, we lost $125,000 in federal grant funding for our care coordinator
program.

This program is part of what distinguishes our
services from a typical doctor’s office. 
Our care coordinators meet with every new patient to assess their social
drivers of health – things like housing and food security, safe relationships, stress,
and other factors in people’s lives that go beyond the scope of traditional
primary care but can be root causes of health problems.

When a patient has one of these underlying problems,
our care coordinators will work with them to find a solution.  They connect them with other local
organizations that can help, assist with applications, help them think through
next steps, and offer encouragement and support through setbacks.  For patients going through a difficult time,
our care coordinators are the people in their corner, helping them get to the
other side. 

One longtime patient we talked to has used the care coordination program to support her through deteriorating eyesight that left her unable to continue her work. She explained that the care coordinators suggested that she might want to apply for disability. "They helped me through that whole process, which was a nightmare. I would have never been able to do it on my own. Oh, I don’t know where I would have gone. These people are saving my life at this point. They kept me going, they kept finding me the next step. They walk me through all of my mail, all of my Medicaid, Medicare – all that stuff, you need to know what’s going on. They’re just helpful. Even if I could see, that stuff to me is over the moon."

This
approach to healthcare means our patients will have better outcomes – but it’s
also just the right thing to do for people in our community who are
struggling.  We
are so gratified to be able to do this work, and we intend to continue despite
this funding setback.  But we could use
your help.

Every
dollar you contribute this NH Gives Day will go to support our care coordinator
program.  It will help
ensure that when one of our patients is having a hard time, we have staff who can
provide the support needed to fix what’s going wrong and help them reach a more
stable and healthy place.
