a division under the Department of PublicWelfare, funds consumer-directed personal assistance services for Medicaid eligiblepeople with disabilities to maintain independent in their own homes. Under thisprogram, consumers choose service coordinators to assist them in complying withregulations under this program. CIL Opportunities is a current provider ofservice coordination which includes empowering consumers to achieve theirself-identified IL goals along with providing personal emergency responsesystems (PERS), home modifications, and durable medical equipment (if deemedeligible by OLTL).CILOpportunities also provides advocacy for con

sumers (regardless if chosen as aservice coordinator) applying for HCBS services.Nursing Home Transition..........................

with IL goals of transitioningout of nursing facilities/state hospitals. This is an unfunded service of whichempowers consumers to live the American Dream … independence. Staff work withconsumers in their current living situation on skills building and independencegoals in preparation for moving day. We also advocate for consumers who haveapplied for the nursing home transition program through the PA Department of Aging.CILOpportunities collect household items from individua
