Partnering with individuals and families to encourage independence through community-based solutions.

Central Nebraska Community Action Partnership (CNCAP) Family Outreach Department offers a variety of programs that promote the strengths in families by providing the following; assistance to families in crisis; linkage to resources; advocacy for services; financial counseling; and attainment of goals. Family Outreach Coordinators and Case Managers are also available to give community presentations to inform individuals and groups on the following emergency issues and trends such as: CNCAP Programs; local, state, and federal resources; community needs; and consumer counseling services.

PROGRAMS

SSVF Program is designed to assist homeless or near homeless Veterans, both individuals and families, to get access to housing and services. The goal of the SSVF program is to provide services and case management to focus on obtaining stable & permanent housing solutions. 

Thrives program provides rapid-re-housing for the homeless. Assistance is provided to help the participant obtain and maintain affordable housing in their service area.

KIND is a program that provides transitional and/or rapid re-housing for homeless youth. Assistance is provided to help the participant obtain and maintain affordable housing in their service area.

VRRH program is designed to ensure qualifying Veterans, preferring those with a disabling condition, can secure safe and affordable housing. 

SSG Fox Program - Veteran Suicide Prevention Grant Program

This program aims to help provide resources toward community-based suicide prevention efforts to meet the needs of Veterans and their families through outreach, suicide prevention services, and connection to VA and community resources. 

HART is a program that is designed to prevent homelessness or to rapidly re-house those who are literally homeless

Commodity Supplemental Food Program 

Seniors who qualify will receive a monthly food package with a variety of nutritious foods, such as unsweetened or lightly sweetened canned fruits, low sodium or no salt added canned vegetables, low fat dairy products, whole grain cereals and pasta, and a variety of protein foods including lean meats, poultry, fish, and beans.
