Successful community hospitals grow and prosper through community partnerships and philanthropic support. We are asking for the community's help to address social determinants of health in three areas: Building Healthy Communities Fund, Medical Indigent Fund and Medical-Legal Partnership Fund.

Each of these funds provides unique support to address health-harming circumstances that our community members, friends, and patients often face. Building Healthy Communities Fund
- Community projects based on community member needs to address inequities that impact health and educational access
- funds support various lifesaving initiatives including free flu shots, a heart health fair, free mammograms, annual health screenings, school-based health centers, virtual learning labs in low-income areas and more
Supporting this fund is a commitment to keep funds within your community. Your support of this fund allows for Christian Hospital Foundation to address the social determinants of health that impact our patients and community at-large. Past projects include the establishment of school-based health centers within our local districts, creation of a tutoring hub at a low-income housing complex to access virtual learning and providing meals to home-bound or elderly patients.
Medical-Legal Partnership Fund:
With the busiest ED in the state of Missouri, the Community Health Access Program (CHAP) team supports Christian Hospital patients to take better control of their health and overall wellness by connecting them to community resources and regular, preventative care. As community advocates CHAP employees works in the community and alongside healthcare providers at Christian Hospital to identify and support individuals and their families achieve health management and increased wellness. In order to expand the impact of CHAP we are raising funds to establish a Medical-Legal Partnership (MLP). This program will support patients that face health-harming legal issues. With legal-ease patients can eliminate barriers that negatively impact their health access and overall wellness.
Medical Indigent Fund:
This fund provides financial assistance to lower income, underinsured and uninsured patients with access to necessary medications, devices or services they need to stay healthy. Since its inception, donors to this fund have supported over 650 patients receive 1,200 vital medications.
In cooperation with the Medical Indigent Fund, the Meds to Beds Program allows us to teach patients how to take their medications. This service in conjunction with the Medical Indigent Fund allows vulnerable patients to leave the hospital with medications empowered to tackle their health management. These services, supported by the Medical Indigent Fund and Meds to Beds Program, have seen patients take better control of their health which has decreased emergency visits and admissions.
