Community Nurse Connection is a convening organization bringing together people, ideas and resources to improve the quality of life for older adults.

Community Nurse Connection aims to unite, strengthen and support Community Nursing Programs.

For Older Adults who have compromised health, limited finances, and/or scant social support, the mainstream United States health care system provides limited assistance. Often families, friends and local social service organizations try to fill the gaps of unmet needs, with varying degrees of success. Thus, a challenge in many communities is how to help older adults who are experiencing difficulties aging at home to attain a higher level of physical, psychological and social wellbeing. Community Nurses are an important part of the solution!

We believe every community in New Hampshire deserves access to a nurse visit in their home.

Who We Are

The Upper Valley Community Nursing Project helps older adults receive continuity of care and remain independent—at home and in their community.

Our project is based on a holistic philosophy of nursing, which dates back to the mid 19th century—a time when nurses focused on the health of individuals, families, and groups within a community.

Our focus is on the health of older people drawn together by such commonalities as location, religious affiliation, social connections, or fraternal association.

Our goal is to add a health professional to the informal network of volunteers already providing care for older adults, and to facilitate the placement of a nurse in a community to address non-acute needs at home.
