About Community Uplift Program

Community Uplift Program was founded on the idea of inclusivity with emphasis on providing support, resources, and assistance in areas of mental health, recovery, housing stability, and more for anyone in the community. We currently offer care coordination and certified peer support services through Community Connect, Free Through Recovery, 1915(i) Care Coordination, and 1915c Service Management. 

We believe that no matter who you are, where you come from, what you look like, or how much money you have, it shouldn’t prevent you from having a welcoming place to support you in challenging times. We believe in a person-centered approach and meet you where you’re at, no matter what phase of your journey you’re in.

Mission and Vision

Community Uplift Program’s mission is to be an agency that provides recovery supports to anyone struggling with substance use and/or mental health and is a proudly LGBTQ+ affirming space.

Community Uplift Program’s vision is to be an LGBTQ+ affirming space (center) for holistic wellbeing for the community to access person-centered resources and supporting them to be successful on their own terms.
