Heartland Health Center established its first clinic in 2014, providing healthcare to individuals of all backgrounds. The team began with a medical clinic and have since added a dental clinic and quick care clinic. In addition, patients can be seen for behavioral health services.

Heartland Health Center can provide services to patients with private insurance, Medicaid, Medicare, and those without insurance. A sliding fee discount scale is offered to patients based on household size and income.

As a health center, Heartland aims to be a patient-centered medical home. By integrating services from a variety of disciplines, providers can collaborate and address patients' whole-health.
