The Granite State has the second-oldest population in the
nation. Currently in New Hampshire, one out of every five of us is over the age
of 60, and this trend is set to continue. Our programs help to support folks
with long-term care options. Most folks would like to remain living in their
homes and communities. Remaining at home also helps to reduce costs to our state and
community as the average annual cost of a nursing home in NH is $114,000/yr.  The following is a listing of what programs we have available at NH Care Collaborative to help support our NH families:

AGING AND DISABILITY RESOURCE CENTER (FORMERLY SERVICELINK):

The Aging and Disability Resource Center provides unbiased, up-to-date information and assistance to
help individuals and families make informed choices about health care and other
services for older adults, people with disabilities, family caregivers,
Veterans, service members, and their families regardless of income. Every year
our office holds open enrollment assistance from Oct. thru Dec. for those who
need to make a change to their Medicare drug company. We also provide monthly New to Medicare
workshops.

CHOICES FOR INDEPENDENCE:

The NH Care Collaborative provides Care Coordination and
Case Management services for clients eligible for the NH Choices for
Independence (CFI) program. CFI is a New Hampshire Department of Health and
Human Services program for adults 18 years or older who have nursing home level
of care needs but prefer to live at home and are financially eligible for long term
care Medicaid. Some of the support includes Adult Med Day Services, Home
Health Aides, Nursing, Meals on Wheels, Environmental Accessibility Adaptations
(Ramps, Grab bars), Personal care services, and a Lifeline

CAREGIVER SUPPORT: We help assess needs, access local
programs and services and connect caregivers with caregiver support groups. We
also offer education/training to help with caregiving skills; assist family
caregivers to explore respite / home modification options and provide regular
follow up. In addition, we have some limited Caregiver respite funds.

VETERAN DIRECTED CARE: Qualified Veterans with medical needs
work with a VA Medical Center social worker and locally based care coordinator
from our agency to develop a person-centered plan and budget that allows them
to self-direct their home and community-based long-term services and supports to continue living independently at home. Support includes the ability to pay
a family member or friend to provide home care support and environmental
adaptations. 

Your support will help us to fill in some of the gaps not covered by other funding.  Some examples include dental care, home modification, respite care, and transportation support.
