At the North Country Health Consortium, we are working hard to make
North Country communities healthier. But we can’t do it alone—we need

Every day, our Community Health Workers (CHWs) walk alongside our neighbors—connecting them to food, housing, medical care, and compassionate support. Whether it's guiding a new mother through postnatal care, helping someone navigate substance use disorder and mental health recovery, or making sure no one falls through the cracks, CHWs are the lifeline many rely on.

But with recent cuts to funding, many of these critical services are now at risk. We’ve had to reduce or pause support in areas where need is still growing. That’s why we’re turning to our community—because your support can help fill the gap and ensure no one is left behind.
