VNHCH has evolved from a tiny staff of several dedicated nurses to a significant local provider of skilled nursing and
rehabilitative services, long term in-home care, hospice and community wellness
services. VNHCH
provides important in home services, as well as Hospice care for the elderly,
chronically ill and disabled in their homes. 

People want to remain independent
and in their own homes for as long as possible. More advanced medical
technologies can now be provided in the home. As the community faces an aging
population coping with multiple chronic illnesses, in-home services need to be
integrated, efficient and focused on the client and their family.

Visiting
Nurse Home Care & Hospice of Carroll County and Western Maine is a
501(c)(3) non-profit organization, licensed by the states of New Hampshire and
Maine, is a Medicare Certified Home Health Agency and a Medicare Certified
Hospice Agency that has strong collaborative relationships with local and
regional health care providers and hospitals.

Visiting Nurse Home Care & Hospice (VNHCH) offers Skilled Services, including RN's, Behavioral Health Specialists, Chronic Disease Management, and Physical, Occupational & Speech Therapists; Hospice Care to make it possible to be at home during life’s final journey; and Long Term Care Services including help with daily living activities, personal care and routine homemaking tasks, all supervised by a professional Registered Nurse.

VNHCH is Medicare Certified and Licensed in NH & ME. As an independent non-profit agency, your support helps guarantee a better tomorrow for people of all ages in our community .
