Organizational Profile 

Family Support Circle (FSC) was founded in 2001 by caring individuals who recognized the need for quality respite care and in-home support services for individuals with developmental disabilities, and youth development. The organization originally started in Philadelphia Pennsylvania and now in the Metro Atlanta Area. FSC is a 501 (c)(3), nonsectarian, social service agency that benefits the community at large by enhancing the potential and quality of life of each person and families we serve. Within FSC’s person-centered philosophy, is a deep commitment to be the provider of supports with specific expertise in youth development, respite care, in-home support and community access program for both youth and those individuals with Developmental Disabilities. Currently, Family Support Circle, Inc. provides the following programs in two states, Georgia and Pennsylvania: 

 

  • Youth Excellence-Afterschool and Summer Enrichment Programs
  • In-Home Support-Home and Community-Based - short-term and long-term support services
  • Community Access Program (CAP)
  • Abstinence Education-Sexual Risks Avoidance
  • Insight Mentoring Alignment Program (iMAP)
  • Scholarship center
  • Employment Support

Family Support Circle (FSC) works to provide the type of support services that moves towards an ever-improving quality of life centered in a healthy and diverse community.

FSC commits to working side by side with each person, families, agencies, diverse communities, and government entities, to set the standard for measurable outcomes so that those embraced in our Family Support Circle, Inc. shall be free to live their lives and pursue their dreams beyond the limitations of developmental disabilities as well as other mental and emotional illnesses.

Person Centered Planning (PCP)

Goal

 Using Person Centered Planning model (PCP), FSC meets the identified needs of youths and individuals with disabilities to help create a “life well-lived”

Objectives

 Person Centered Planning model helps FSC to identify the supports and the skills that youth and individuals with disabilities need in order to achieve a “life well-lived.” With Person Centered Planning model both youth and people with disabilities can reach full potential with:

  • Lifelong learning activities
  • Making choices & Decisions
  • Medical Support needed
  • Behavioral Support needed
  • Community living connections
  • Home living - life skills
  • Employment focus
  • Health & Safety awareness

 FSC Youth Excellence Program Mission (YEP)

 

The mission of the FSC-Youth Excellence Program is to empower every young man and woman to achieve his/her full potential and personal goal and to enable them to become vital contributors to our complex global society.

Goal

Our goal is to make a difference in every young person's life, one at a time. One by one, we can build communities of healthy, happy, well-educated, career-minded, confident, self-reliant, and successful young people.

Vision

The FSC Youth Excellence program is committed to helping young men and young women develop their fullest potential intellectually, physically, socially, emotionally, and spiritually. To encourage and support one another, respectful of one another's unique talents and interests, aware of, and accepting of cultural, racial, religious, and economic backgrounds that may be different from their own and to prepare youth not only for positive college experiences, but for lifelong opportunities.
