Renewal Life Community Development Corporation operates an 18‑month Transitional Housing Program on the South Side of Chicago, serving individuals experiencing homelessness with a holistic, trauma‑informed approach. The program provides safe overnight shelter, intensive case management, mental health support, and life‑skills development, with the goal of helping participants transition to stable housing, improved mental and emotional health, and sustainable employment.

The program is housed in our 3,000 sq ft sanctuary and can serve up to 50 individuals at a time. Participants typically remain in the program for up to 18 months, allowing sufficient time for stabilization, healing, and long‑term planning.

Target Population

The program serves adults experiencing homelessness in the Southside and Back of the Yards communities of Chicago. Many participants face multiple barriers, including trauma, mental health challenges, substance use histories, unemployment, and lack of family or community support.

  • No government‑issued ID is required for entry.
  • The primary requirement is a willingness to receive help and participate in services and training.

Core Services Provided

1. Safe, Stable Housing

  • Overnight shelter in a safe, structured, and respectful environment
  • Bedding provided for all participants
  • Clear rules to maintain safety and dignity: no drugs, alcohol, profanity, or weapons

2. Food and Basic Needs

  • Hot meals provided twice weekly for the homeless community
  • Access to basic necessities and hygiene support as available

3. Intake, Assessment, and Structured Reviews

  • Intake interview and assessment completed within 72 hours of entry
  • Participants are assessed across spiritual, emotional, and mental health domains
  • Individualized service plans are developed using SMART goals (Specific, Measurable, Achievable, Relevant, Time‑bound)
  • The program includes up to three formal 6‑month reviews (at approximately 6, 12, and 18 months) to:
    • Evaluate progress toward goals
    • Adjust services and supports
    • Identify emerging needs and barriers

4. Mental Health and Counseling Services

  • Access to two licensed counselors (CADC, LPC)
  • Group sessions, individual counseling, and pastoral counseling
  • Counselors available 7:00 AM–3:00 PM, with 24‑hour availability for sessions as needed

5. Life Skills and Personal Development

  • Coaching in goal setting, time management, and daily living skills
  • Trauma‑informed, strengths‑based, and holistic support
  • Flexible, adaptive plans that evolve as participants progress

6. Employment and Transportation Support

  • Wrap‑around services, including transportation assistance
  • Connections to job fairs and employment opportunities
  • Support in building sustainable income and preparing for independent living

Program Approach and Philosophy

The 18‑month Transitional Housing Program is grounded in a client‑centered, strengths‑based, and trauma‑informed framework. Renewal Life Community Development Corporation views each participant as a whole person—body, mind, and spirit—and designs services accordingly.

Key principles include:

  • Dignity and Respect: Every participant is treated with honor, regardless of their history or current circumstances.
  • Client‑Guided Goals: Participants actively shape their own goals and definitions of success.
  • Holistic Care: Spiritual, emotional, mental, and practical needs are addressed together, not in isolation.

Outcomes and Success Measures

Success is measured based on the individual goals participants set in collaboration with staff and counselors. Outcomes may include:

  • Transition to permanent or more stable housing
  • Employment or increased income through job placement or training
  • Improved mental and emotional health, as reported by participants and clinicians
  • Increased life skills, self‑efficacy, and community connection

The three 6‑month reviews provide structured checkpoints to document progress, refine goals, and ensure that services remain aligned with participant needs throughout the 18‑month period.

Need for Support

Funding is critical to sustain and strengthen this program, including:

  • Bedding and basic supplies for up to 50 individuals
  • Support for licensed counseling services and program staff
  • Building operations and repairs to maintain a safe, stable environment
  • Expansion of partnerships and wrap‑around services to reduce early departures due to unmet resource needs
